People Creating Success, Inc. works in concert with the individual’s circle of support to facilitate and sustain their liberty to make life decisions based on choice, personal preferences and need. Individuals are encouraged to make informed choices, get involved in their immediate community and become as independent as possible, all of which will contribute to increased self-esteem and a richer quality of life.
Support is based on each individuals need and desires. We firmly believe that considering each participant as a unique individual is the most effective way of helping people achieve personal goals and safely increase their independence to the maximum potential. People are involved in all aspects of planning their days so that they will be successful, the use of person centered tools helps all members of an individual’s team gain an understanding of who the person is and what type and level of supports will encourage them to grow and obtain their desired dreams.
PCS believes that any person can be successful in a Supportive Living environment. A strong sense of community acceptance and use of natural and/or generic resources benefits each person as they gain independence. We promote the use of community resources and connecting people with other community members and resources. This approach helps build a life-long support system that will benefit people for years to come.
Independent Living Services are ideal for people who are relatively independent and need only a few hours a week of assistance. We tailor our service to the individual to optimize the time of service to be the most beneficial to them, whatever their needs may be.
Independent Living Services (ILS) are ideal for people who are relatively independent and need only a few hours a week of assistance. PCS tailors a unique program for each individual so they receive the optimum benefit out of the hours of service. ILS hours are designed to be utilized on a 1:1 basis and can be authorized for up to 20 hours per month per person. Additional hours may be approved by the Regional Center on an exception basis. Hours of support are usually utilized to provide assistance with money management, bill paying, medical appointments, community connections, social integration and home maintenance. Individuals can start with ILS support or transition from SLS support to ILS support if/when their support needs to change and their independence grows.
PCS provides ILS in Santa Barbara, Ventura County, Antelope Valley and Northern Los Angeles.
Respite: The primary purpose of the PCS In-House Respite Services PRogram is to provide temporary relief to a regular caregiver of an indivdidual with a developmental disability. By providing respite we will assist family members in maintaining the indivdiual at home, provide basic care and supervision in the absence of the family, relieve family from the demands of care giving and attend to the individual’s basic self help needs and other activities of daily living including interaction, socialization and continuation of their regular daily routines.
PCS provides Respite care in Santa Barbara and Ventura regions.
PCS provides Specialized Adult Residential Services in Ventura and North Los Angeles Counties. The homes are designed to serve individuals from Developmental Centers that might otherwise not have a chance to move into a community setting.
PCS operates three Adult Residential homes. Two of the homes are located in Ventura and the third home is located in West Hills, California. The development of the homes was an effort to provide alternative community transition options to individuals residing in the Developmental Centers.
The Ventura homes are Specialized Residential homes with a medical and behavior component that helps to meet the needs of the people residing in the homes. Each home has a four bedroom layout and each person has their own room. Our West Hills home is structured as an Adult Residential Home with four male individuals, all who moved from the Developmental Center System into their community setting.
The homes operate under the philosophy that each individual has the right to choose how their day is spent, who their staff is, and what community participation they are interested in. Employees working in the homes meet the requirements and experience level to work effectively with each individual.
A unique benefit for the homes is the infrastructure system that ensures all needs are met regarding behavior and medical challenges. PCS employs a full time Licensed Vocational Nurse who visits the homes at a minimum of 5 days per week and oversees all medical components of an individual’s care. In addition to the LVN, PCS utilizes several consultants including a Registered Dietician, Behavior Specialist, Visiting Doctor and Nurse Practitioner.
The PCS approach is to facilitate as much independence as possible for residents using a Person Centered format. Adaptations and accommodations will be made to facilitate learning and comfort.
FADE- Facilitating Alternative to Day Environments
The mission of the FADE program is to give individuals with developmental disabilities the opportunity to participate in the typical career, education and life goals that most adults pursue. Our goal is to give individuals the skill set necessary to have a meaningful life of their choosing – whether that be a career or a life of service. Once that individual has the skills and confidence necessary to pursue their own career/life path, paid supports will fade out.
FADE offers two distinct tracks: the Development Track (to assist the individual to find their purpose and career goals) and the Achievement Track (to assist the individual in creating natural supports once the individual has defined their personal life goals).
An individual may not know “what they want to do with their life.” They may not have had the opportunity to think about a career and what their lives could look like in the next five years. This individual would be on the Development Track. This track focuses on career goal development and future planning. This track will teach self awareness, choice, social skills, volunteer opportunities, education, pre-employment skills, and independence training
If the individual clearly has a career/life path in mind then FADE focuses on how to best assist the individual to achieve that goal and how to best wean-off paid supports. This individual will be on the Achievement Track. This track focuses on the implementation of goals specific to the individual’s career and finding natural supports to replace paid supports. This track will emphasize education, PASS plans, business plans, marketing, professional skills, self esteem/confidence, and independence training.
